Aspergillose - invasief

anidulafungin iv loading dose 200mg, 100mg 1dd
Remarks:

Voriconazol eerst opladen: dag 1 6mg/kg 2dd, gevolgd door 4mg/kg 2dd en daarna op geleide van TDM.

Alternatieven voor anidulafungin zijn caspofungin (70 mg oplaad, 50 mg 1 dd iv) of micafungin (100 mg 1 dd iv).

voriconazol iv or po 4mg/kg 2dd

amfotericine B liposomaal (Ambisome) iv 3mg/kg 1dd
Remarks:

Bij (mogelijk) gemengd azol-gevoelige/resistentie stammen of (mogelijke) dubbelinfectie met mucorales.

Voriconazol eerst opladen: dag 1 6mg/kg 2dd, gevolgd door 4mg/kg 2dd en daarna op geleide van TDM.

Voorkeur voor liposomaal amfotericine B (Ambisome) boven conventioneel amfotericine B (Fungizone) vanwege risico op (nefro)toxiciteit.

Indien contra-indicatie voor azool.

Remarks:

Bij contra-indicaties, intolerantie of bijwerkingen van voriconazol of isavuconazol EN aangetoonde gevoeligheid.

Dosering posaconazol hangt af van formulering:
Therapeutisch:
iv: eerste dag 2 dd 300 mg, daarna 300 mg 1 dd
po: Bij voorkeur tabletten: eerste dag 2 x 300 mg, daarna 300 mg 1 dd po. Alleen indien tabletten onmogelijk: suspensie: 400 mg 2 dd po (met voedsel) of 200 mg 4 dd (zonder voedsel).

Bij chirurgische resectie: peri-operatieve profylaxe: voriconazol 2 dd 200 p.o. van 2 dagen vóór tot 3 dagen na de ingreep. 
Bij inoperabele patienten arteriele embolisatie en/of intra-laesionaal conventioneel amfotericine B overwegen.

Indications: Secundaire profylaxe bij primaire immunodeficiëntie (bv CGD) of bv. HSCT
