Remarks:

Voriconazol eerst opladen: dag 1 6mg/kg 2dd, gevolgd door 4mg/kg 2dd en daarna op geleide van TDM. Dosering bij p.o. behandeling: dag 1 400mg 2dd, gevolgd door 200mg 2dd en daarna op geleide van TDM

Remarks:

Bij (mogelijk) gemengd azol-gevoelige/resistentie stammen of (mogelijke) dubbelinfectie met mucorales

Voriconazol eerst opladen: dag 1 6mg/kg 2dd, gevolgd door 4mg/kg 2dd en daarna op geleide van TDM. Dosering bij p.o. behandeling: dag 1 400mg 2dd, gevolgd door 200mg 2dd en daarna op geleide van TDM 

Voorkeur voor liposomaal amfotericine B (Ambisome) boven conventioneel amfotericine B (Fungizone) vanwege risico op (nefro)toxiciteit

iv: eerste dag 2 dd 300 mg, daarna 300 mg 1 dd
po: Bij voorkeur tabletten: eerste dag 2 x 300 mg, daarna 300 mg 1 dd po. Alleen indien tabletten onmogelijk: suspensie: 400 mg 2 dd po (met voedsel) of 200 mg 4 dd (zonder voedsel)
