General comments
Algemeen: een acute balan(opost)itis kan veroorzaakt worden door:
- Infectieus, meest voorkomend: groep A- en B-streptokokken. Daarnaast: Candida albicans en andere Candida spp., anaërobe bacteriën (Gardnerella vaginalis, Bacteroides melaninogenicus), spirocheten en SOA's (Trichomonas vaginalis, Chlamydia trachomatis, Neisseria gonorrhoeae, syfilis en HSV).
- Niet-infectieuze oorzaken, zoals trauma, irritatie en/of allergische reactie op lokaal aangebrachte stoffen.
Een chronische balanitis wordt zelden veroorzaakt door een infectie, met uitzonder van Candida-infecties (vooral bij mannen met diabetes mellitus of een verstoorde weerstand).
Aanvullend behandeladvies:
- Wassen met ruim lauw water of een NaCl-oplossing 0,9%, geen zeep gebruiken in het genitaal gebied.
- Alternatief voor milde anaërobe balan(opost)itis is clindamycinecrème, 2 dd tot klachten zijn verdwenen.
- Bij chronische of recidiverende balanitis al of niet als gevolg van phimosis die niet goed reageert op conservatieve therapie moet circumcisie overwogen worden.
